Needs Assessment:
There will be an estimated 3.5 milling total knee arthroplasties performed by 2030. Out-patient surgery traditionally reduces cost by up to 30%, so there is a huge potential for cost savings. Recent advances in surgical technique. anesthesia and protocol driven care have allowed this procedure to potentially be performed with a shorter length stay. It is desirable to try and reduce length of stay to try and minimize health care costs, without increasing readmission rate, and complications. Identifying patient factor which are associated with successful date case knee arthroplasty is important. Although "multi-modal analgesia" is a current trend in anesthesia care, the use of adjunct medication is not consistent. This is especially true concerning ketamine. This is related to contradictory information and teaching, both past and present:
Various publications suggest perioperative ketamine may reduce acute opioid requirements. But the protocols are doses differ widely
Anesthesiologists are most familiar with ketamine as a sedative/hypnotic/anesthe
tic
At moderate dosed ketamine use is linked to dysphoria and agitation
Low dose ketamine has limited effect in many studies
Ketamine use is often not allowed in hospitas outside of the operative/procedural areas
Course Objectives:
Evaluate the elements of a protocol based pathway aimed for knee arthroplasty
Identify patient factors associated with successful day case joint replacement
Discuss the barriers to day case joint replacement surgery
List the various activities of ketamine at different doses and infusion schemes
Identify the unique properties of ketamine that can be used to modify opopod requirements, toleratnce, and hyperalgesia in the perioperative period
Develop an active plan to use ketamone in perioperative period to reduce opioid requirements
Define point of care viscoelastic testing, and its uses, both Rotational-Thromboelastog
raphy (TEG) and Thromboelastometry (ROTEM)
Identify clinical scenarios where th euse of TEG and/or ROTEM is appropriate
Describe how to use viscoelastic testing for targeted coagulation management massive transfusion events
Review TEG and ROTEM analyses and graph interpretation
