Registration:

This activity is designed for Pharmacists of the live conference who want to claim ACPE contact hours and/or CME Credits.

  • For information on this course, click here to visit the UCLA CME course home page.
  • This activity will generate a certificate at the end of the process. This certificate is for AMA PRA Category 1 Credits TM only.
  • Regarding CPE: Instructions, Universal Access Numbers, and Learning Objectives are available from within this activity.
  • Each Lecture = Session. Keep in mind that some sessions run concurrently and cannot be selected together.
  • Only select the lectures/sessions that you have attended. If you select the wrong session by accident, please contact Pedro Torres.
  • Participants have 45 days from the end of the conference to complete assessments and receive your CPE credit.
  • CPE credits will be electronically submitted to CPE monitor through the NABP website.
  • If you need to print a statement of attendance you may do so there.
  • ACPE does not allow us to issue statement of credits or attendance.
  • If you complete the online assessments for the CE session within the deadline, your CPE credits will be uploaded to the CPE Monitor System within 10 days of completion. If you do not see CPE activity displayed for a session that you attended and it has been more than 10 days since you submitted the CPE to the provider, please contact: UCSF Office of CME, 415-476-4251, info@ocme.ucsf.edu

Instructions:

This activity consists of a number of surveys/assessments that are relevant to the Intensive Geriatrics course. To earn credits from this activity, participants must complete:

  • Course Pre-Test (General Survey)
  • Lecture/Session Post-Tests (One for each of the lectures you attended)
  • Course Faculty Evaluations (One for each day of the conference)
  • Course Post-Test (General Survey)
  • Course Overall Evaluation (General Survey)
  • Course CME Verification Form
